sign-in of your examination,
Bring two (2) passport photos attached to your
admission letter,
A kit for the examination,
Must wear a Lab coat or smock

As of January 2008, the Arizona State
Board does not administrator the practical and written exam at our office. We
have contracted with Professional Credential Services for the practical exam and
LaserGrade Computer base testing for the written exam. Information for both
examinations visit:
